Resonance Music Therapy is committed to protecting the privacy of clients, families, and all individuals whose personal information we collect and hold. This Privacy Policy explains how we collect, use, store, and disclose personal information, and how you can access or correct information we hold about you.

Resonance Music Therapy is operated by Barak Dar-El (ABN: 37 764 425 002) and complies with the Australian Privacy Principles (APPs) under the Privacy Act 1988 (Cth).

What Personal Information Do We Collect?

We may collect the following types of personal information:

 

Contact and identification information:

  • Name, address, phone number, email address

  • Date of birth

  • Emergency contact details

 

Health information:

  • Medical history and diagnoses relevant to music therapy

  • Current medications (where relevant to session planning)

  • Information about existing supports and allied health team members

  • Session notes and clinical documentation

  • Assessment reports and progress reports

  • Goals and care plan information

 

Funding information:

  • NDIS plan details and participant numbers

  • Support at Home classification and provider details

  • Private billing and payment information

 

Other information:

  • Photographs or videos taken during sessions (only with explicit written consent)

  • Information shared by family members, carers, support coordinators, or care managers

 

We collect only the information necessary to provide safe, effective, and person-centred music therapy services.

How Do We Collect Personal Information?

We collect personal information:

  • directly from you or your authorised representative through our contact form, phone conversations, and Service Agreement

  • from referrers such as support coordinators, care managers, or allied health professionals (with your consent)

  • through clinical observations and session documentation during the course of service delivery

How Do We Use Personal Information?

We use personal information to:

  • deliver music therapy services tailored to your goals and needs

  • communicate with you and your authorised representatives about appointments and services

  • prepare clinical assessments, session notes, and progress reports

  • coordinate with other members of your care team (with your consent)

  • process invoices and manage billing through NDIS, Support at Home, or private arrangements

  • meet our legal and professional obligations under AMTA standards, the Aged Care Quality Standards, and the NDIS Practice Standards

How Do We Use Personal Information?

Client information is stored digitally using Google Workspace (including Google Drive and Google Sheets), which is protected by password access and two-factor authentication. Google Workspace operates in accordance with industry-standard security practices and relevant data protection requirements.

Physical documents containing personal information (such as signed Service Agreements) are stored securely and disposed of appropriately when no longer required.

We retain client records for a minimum of 7 years from the date of the last service, or in the case of a child client, until they reach the age of 25 — in accordance with professional and legal obligations.

Who Do We Share Personal Information With?

We may share personal information with:

  • Support coordinators and plan managers — to coordinate NDIS-funded services and submit invoices

  • Care managers and Support at Home providers — to coordinate aged care services, provide quotes, and submit claims

  • Other allied health professionals — where you have consented to information sharing for coordinated care (for example, sharing a report with a speech therapist or occupational therapist)

  • AMTA or relevant regulatory bodies — where required for professional oversight or complaint investigation

  • Government agencies — where required by law (for example, mandatory reporting obligations)

 

We do not sell, rent, or share personal information for marketing purposes.

Photography and Video

Photographs or videos are only taken during sessions where explicit written consent has been obtained through the Service Agreement. Consent is specific — you may consent to photography for internal clinical purposes without consenting to social media use, or vice versa.

Even where general consent has been given in the Service Agreement, we will always ask for verbal confirmation before taking photographs or video in any individual session.

You may withdraw consent for photography or video at any time by contacting us in writing.

Your Rights

You have the right to:

  • request access to the personal information we hold about you

  • request correction of inaccurate or incomplete information

  • make a complaint about how we have handled your personal information

  • withdraw consent for specific uses of your information (such as photography) at any time
